2019 Community of Faith Youth Nomination Form

  • THE SHINING WATERS REGIONAL COUNCIL INAUGURAL REGIONAL MEETING May 24th – 26th, 2019

    The inaugural regional meeting will take place at Georgian College in Barrie. The Youth and Young Adult participants will gather on Thursday, May 23rd at 6 p.m. in the lobby of Georgian College.

    SUBMISSION DEADLINE: Monday, April 8, 2019

  • Please enter the city or town where your community of faith is situated.

  • CHAIR OF COMMUNITY OF FAITH GOVERNING BODY (OR DESIGNATE)

    By endorsing this form, I confirm that this nominee has the endorsement of our community of faith and that they:

    • Would like to participate in the Shining Waters Regional Council inaugural regional meeting;

    • Are able to attend from Thursday May 23 – Sunday, May 26; and

    • Have permission from their parent/guardian to participate (if under 18 years of age).

    This year there will be a bus travelling from Toronto to ensure that all youth and young adults have transportation to the meeting. Further information about the bus pick up and drop off will be updated as it becomes available.

    If the participant does not come by bus they are required to arrive by 6 p.m at Georgian College in Barrie.

    Transportation home will be provided back to the same locations in Toronto. Participants will need to arrange their own way to these drop off locations. If not, it will be the responsibility of the parents to arrange travel plans if the participant chooses not to travel by bus (Participants 18 and older may make their own travel arrangements).

  • This Nomination Form should be submitted online by the Chair (or designate) of your Community of Faith by Monday, April 8, 2019
